BEFORE GIVING COMMUNITY SERVICE:

  1. See information about types of service on Outreach Main Page.

  2. Complete and submit the information in the box below (must submit a separate form for each DIFFERENT place of Outreach service)

  3. After submitting, you will  see a new screen containing  COMMUNITY SERVICE CONFIRMATION PAGE

  4. Save and print the confirmation page and follow all instruction listed there.

  5. Forms are due within 30 days of giving service; forms for service completed during  summer are due by September 20.
